Due to the outbreak of Coronavirus (Covid-19), Magic Little Moments is taking extra precautions, including enhanced sanitation/disinfecting procedures and compliance with the UK Government guidance, with the care of every client in mind.

 

Symptoms of Covid-19 Include:
* Fever or high temperature

* New, continuous cough

* Change in sense of smell and taste

 

I agree to and acknowledge the following:

• I understand the above symptoms and affirm that I, as well as household
  members, do not currently have, nor have experienced the symptoms listed above within the last 14 days.

• I affirm that I, as well as all household members, have not been diagnosed with COVID-19 within the past 14 days.

• I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 14 days.

• I affirm that I, as well as household members, have not traveled outside of the country to any place considered to be a “hot spot” for COVID-19 infections within the past 14 days.

• I understand that Magic Little Moments cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form

• I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the UK government and authorities recommend practicing social distancing. 

• I acknowledge that Magic Little Moments has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

• I acknowledge that Magic Little Moments can not guarantee that I will not become infected with the Coronavirus/Covid-19. 

• I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others.

 

By attending our sessions, you agree to each statement above and release Magic Little Moments from any and all liability for unintentional exposure or harm due to COVID-19.
